After a season of utter dominance, Chelsea and Jose Mourinho will greet their fans on bank holiday Monday as they tour the streets surrounding Stamford Bridge to parade the Premier League trophy and Capital One Cup.

Chelsea have cantered to the league title this season, with no side able to hold on to their coattails.

The Blues senior side will be joined by their highly successful academy team, who won the FA Youth Cup and UEFA Youth League—making it collectively one of Chelsea's greatest ever seasons across the board.

It has been a magnificent homecoming for Jose Mourinho who, since his return from Real Madrid to English football two years ago, has fine-tuned his Chelsea machine into England's greatest once more.

On Monday, the Blues can celebrate their achievements with their adoring public as the supporters of the famous west London team line the streets to give praise to their victorious men.

Chelsea TV released a video detailing the full route of the parade, allowing fans to pick their viewing spots with ease:

Proceedings will begin at Stamford Bridge and are scheduled to get underway at 12 p.m. as announced by Chelsea's official website. They will head east up Fulham Road, past Saint Mark's Chapel to Hortensia Road.

The bus will then turn right to pass Kensington and Chelsea college, finding its way to the local area's most famous landmark of King's Road.

The team will then head back west past the cosmopolitan area's affluent restaurants and eateries, joining on to New King's Road.

Following that, the parade will then conclude at Eel Brook Common, with significant restrictions in place, allowing the festivities to go off without problems.

Mourinho gave thanks to the club's supporters recently, expressing his gratitude to their consistent backing of his side, motivating the team to perform wonders:

Chelsea's fans will feel they suffered in the years when Mourinho returned to continental Europe—despite winning the Champions League—and they will now be satisfied they are genuinely the best team in England once again.

John Terry has rolled back the years to find his best form once more—something many pundits did not think possible just one year ago.

The goal now will be to once again win Europe's elite competition and to add a new set of world-class players to the squad's already diamond-encrusted ranks.

But first the club will celebrate, and Chelsea fans will want to hold onto the Special One's magic touch forever.
